SmartSuite SmartSuite® is a package of benefits your employer offers that provides you a choice of plans as well as guidance in holding down your healthcare costs

High Deductible Health Plan (HDHP) A specially designed PPO or POS plan compatible with the Health Savings Account (HSA)

Personal Care Account PPO A Preferred Provider Organization (PPO) health plan combined with a personal healthcare spending account

CoverageFirstSM Catastrophic coverage plus a $500-per-member benefit allowance for in-network services

Preferred Provider Organization (PPO) A health plan with a wide choice of providers and discounts for using in-network doctors and hospitals

Health Maintenance Organization (HMO) Predictable costs for you and an emphasis on your relationship with a primary care physician (PCP)

Point of Service (POS) Similar to an HMO but with flexibility to seek services outside the network

Humana Classic A plan for members who live outside Humana’s network areas
