How a Classic Plan Works
The Classic plan – also known as a "traditional indemnity plan" or a "fee-for-service plan" – has no provider network. You can choose any doctor or other healthcare provider, and you don't need a referral to see a specialist.
A Classic plan is a good choice if you are willing to pay more for coverage to have the same level of benefits with any provider.
- Your costs are the same for any provider. You don't have to worry about paying more for certain doctors and hospitals.
- You can keep the same doctor. This plan doesn't require you to see in-network doctors.
- You're covered wherever you go. This plan offers flexibility for people who travel or live in an area with few in-network providers.
- Your out-of-pocket costs may include copayment, deductible, or coinsurance.
- If you reach the out-of-pocket maximum, the plan pays 100 percent of additional covered expenses during the plan year. You continue to pay copayments.
- With a Classic plan, you may have to pay for medical services up front and then submit a claim to Humana for reimbursement.
