With the HMO plan, you work one-on-one with a primary care physician (PCP) who coordinates your care and provides referrals to other in-network providers when necessary.* Plus, you aren't locked in once you choose a PCP – you can switch to another in-network doctor anytime.

Humana Gold Plus includes all the benefits of Original Medicare, plus prescription drug coverage and added features like Humana Active Outlook^SM. The SilverSneakers^® Fitness Program is also available with many of our Medicare HMO plans.
