Coronary Artery Disease (CAD) Program

This nine-month program is available to eligible Humana members with high-acuity coronary artery disease (CAD).

Program Overview

Humana contracts with CorSolutions Inc. to manage its CAD program, which seeks to coordinate care and complement the treatment plan prescribed by a member's physician. The program is supported by CorSolutions' disease-specific protocols and specially designed behavior-modification tools.

Program components

  • Proactive identification of candidates for the program through the use of medical data, physician referrals, and precertification information

  • Registered nurses are available via telephone 24 hours a day, seven days a week to answer questions from the patient and the patient's family members

  • Close contact is maintained with the participating physician and the patient to help coordinate care, to provide educational materials, and to improve communication
