Coronary Artery Disease (CAD) Program

This nine-month program is available to eligible Humana members with high-acuity coronary artery disease (CAD).

Program Overview

Humana contracts with CorSolutions Inc. to manage its CAD program, which seeks to coordinate care and complement the treatment plan prescribed by a member's physician. The program is supported by CorSolutions' disease-specific protocols and specially designed behavior-modification tools.

Program components

Proactive identification of candidates for the program through the use of medical data, physician referrals, and precertification information
Registered nurses are available via telephone 24 hours a day, seven days a week to answer questions from the patient and the patient's family members
Close contact is maintained with the participating physician and the patient to help coordinate care, to provide educational materials, and to improve communication
