About Humana's Medication Therapy Management Network

Humana's MTM program began in May 2006.

All MTM-eligible members receive monthly educational mailings that address topics such as compliance, medication profiles, and questions to ask their pharmacist. A smaller population of our MTM members are eligible for either a telephonic consultation or a face-to-face consultation with a health professional. Our MTM pharmacy network includes chain pharmacies, independent pharmacies, consultant pharmacists, and clinical pharmacists who practice in a hospital setting.

Pharmacy Eligibility

Humana utilizes proprietary criteria to determine eligibility for our MTM program. This includes member information such as the number of chronic medications and diseases, anticipated drug costs, and demographic data. Humana's MTM program is available to qualifying members enrolled in either a Part D Prescription Drug Plan or a Medicare Advantage plan with prescription drug benefits. Once members become eligible, they are enrolled for the entire calendar year and re-evaluated the following year. Please review our Pharmacy Eligibility Requirements.
